It's more like sparring matches, yeah- but if things go too far there's medical centers in every city, and medicines you can purchase for when you're on the road. Healthcare is also free- for pokemon or people, so... that's pretty great, actually!

Admittedly, some battles can get pretty intense- there's a local crime group who also rely on pokemon, as all things in this world does sometimes, and... well. A friendly spar and defending yourself from thugs are pretty different.
